Health Visiting Service for Older People

"I am registered nurse with a degree in
Health Visiting. I work in the Community as part of the Primary Health Care
Team, and have expertise in the assessment and identification of health needs of
the older person."
The aims of our service are:
- To
promote healthy lifestyles and prevent ill health.
- The
early identification of illness through health surveillance programmes.
- To
enhance the quality of life of those whose health is impaired.
We will achieve this by:
- Health
Surveillance Programmes.
- Liaison
through multidisciplinary links.
- Providing information and resources.
- Assessment and identification of needs.
- Providing support for carers.
- Counselling.
- Health
promotion - working towards the Government Health of the Nation Targets.
- Profiling (Community/Caseload) - We have a lead role in the identification of
public health needs and working with the multiagency team to improve services.
- Referrals to appropriate statutory and voluntary agencies.
