Chaperones

If you feel you would like a chaperone present at your consultation please let us know either before or during your appointment. One can be provided for you or you can bring a friend or family member.

Don't know your NHS number? 

 

If you live in England you can use this service. You can also use this service for someone else.

They will ask you for your name, date of birth and your postcode. They will then send you a text, email or letter with your NHS number on it.

If you are using the service for someone else, enter their details and they will be sent their NHS number.
